Online Registration

Deadline for registration for the symposium is 5:00 pm (EST), Monday, June 15, 2009. A request for a refund must be received in writing by June 15, 2009. No refunds will be given after June 15, but registration is transferable through June 24, 2009. A $50 cancellation fee will be deducted from the refund for all cancellations.

Please Note:
If you register online, you will receive an immediate registration confirmation notice indicating that payment was processed. If the notice contains the word “error” please contact the registration office at 216-448-0777 to confirm registration. Mailed or faxed registrations will receive a confirmation notice by U.S. mail unless requested otherwise on the registration form.

Registration

This course uses our online registration system which instantly approves credit card transactions and issues you a receipt immediately. 

Instructions for first time users: 

• Login initially by selecting a registration category (Physician, Nurses, etc) from the drop down menu. 

• Provide your personal information (name, address, telephone, and a 4 digit number password of your choice). The password is  required and will serve as your login identification for all future registrations. 

• Select course options and enter payment information in the next screen.

Registration fee includes registration materials, continental breakfasts, lunches, receptions, and refreshment breaks.
