Pre-registrations are accepted by fax or online until 4:00 pm ET May 11, 2009. To register after this date, you must do so on-site.
In case of cancellation, a full refund will be made if canceled by April 30, 2009. After April 30, 2009, a $100 cancellation fee will be deducted from your refund. Written notification of your cancellation is required to process your refund.
NO REFUNDS WILL BE ISSUED AFTER May 31, 2009.

Registration
This course uses our online registration system which instantly approves credit card transactions and issues you a receipt immediately.
Instructions for first time users:
- Login initially by selecting a registration category (Physician, Nurses, etc) from the drop down menu.
- Provide your personal information (name, address, telephone, and a 4 digit number password of your choice). The password is required and will serve as your login identification for all future registrations.
- Select course options and enter payment information in the next screen.
Registration fee includes syllabus, continental breakfast, lunches, refreshment breaks and banquet.
